Improve milestone calendar output: add URL's and rearrange titles for readability

Two things that would make my day:

1. Populate the event URL field with a link to the milestone page in Trac
2. Include the Trac instance name in event titles, e.g. [Instance]: Milestone blah blah. In iCal's week or month view, all text beyond "Milestone" is currently cropped out, and I have a lot a Trac instance for each project.

As far as I can see with current 0.11b1+, the URL is already part of the event fields. #3946 actually reports an error on it.

As for the event titles, I'm not so sure. The name of the calendar itself is fine, and also includes the project name. With all regular use of iCal feeds in popular calendar tools these allow color-coding or other means of distinguishing calendars. If anything, the text should be switched so that it said blah blah (project_name Milestone)? If visual cropping in client is a consideration, that would be the most meaningful order to me at least.
