Description
The current head revision does not have a trac-post-commit-hook. Since it was removed in Trac 0.12 development but is still referenced in the documentation at http://trac.edgewall.org/wiki/TracInstall#AutomaticreferencetotheSVNchangesetsinTractickets I recommend putting the revision before it was deleted in the link.

That section is gone from the 0.12 version (0.12/TracInstall). We could re-add it, but describing the tracopt.ticket.commit_updater plugin instead.

Thanks for fix but I am not sure that this fixes the problem I was stating. What I would like is for the current TracInstall page for 0.11 to have the link for the trac-svn-post-commit-hook file fixed (under TracInstall#AutomaticreferencetotheSVNchangesetsinTractickets). Since it deleted for Trac 0.12 I recommend replacing the link in TracInstall source:trunk/contrib/trac-post-commit-hook@8427.

Oh, sorry about that. Actually, we should reference the version on the 0.11-stable branch in that case (which is identical to the last revision on trunk). Done in TracInstall@295.

Have a first look at TracRepositoryAdmin#ExplicitSync which explains the basics for Trac 0.12.
Then read about TracInstall#AutomaticreferencetotheSVNchangesetsinTractickets, and if you find that interesting, then I hope this might interest you too: TH:TracTicketChangesetsPlugin (I am the author of the plugin, please submit tickets on the Trac-Hacks if you find any problem with it).
