Two buttons have same access key

Description
On editing wiki page, both "Preview Page" button and "Review Changes" buttons have same access key "r". And when I've hit access key 'r' (by SHIFT+ALT+R on firefox), "Review Changes" is performed. Two accesskeys should be differ at least, and I think it'd better to assign 'r' for preview like 0.10.
And also "Merge change" button has same accesskey although (disabled) "Preview" button exist. If 'r' is for previewing, I guess it may confuse users.

Removed the duplicated one in r7425. ありがとうございました。
The duplicates are actually intentional in order to limit the keys used we reassign (potentially overriding the browser's own).
