Ticket description should have a title — at Initial Version

Description
The ticket description shown in the Ticket box should have a Title, similar to the titles for the change, in the Change History below:
- this would make the display of the Reply to description button consistent with the one of the Reply to comment (whether or not it ends up being above or below the line, at least there is a line, now!)
- this makes it possible to easily indicate whether the description is the original one (the title is simply Description), or has been changed (the title becomes Description (Last Modified by xxy))
