tracd does not reload htdigest files

htdigest files are only loaded during the start of the tracd server. changes done later (even removing users) don't change the behaviour of the server. while adding users during the runtime of the server would be a nice thing to have the fact that the server ignores deletion might pose a security problem.

Incidentally, this also reduces the utility of the AccountManagerPlugin when running tracd.
