Please read NewTicketGuidelines about how to report a useful bug/error.

Your bug report is missing an important information: the Trac version.
Moreover, this is a duplicate which is easy to locate: simply search for the error: columns type, name are not unique
BTW, please use verbatim blocks ({{{}}}) when quoting an error trace.
